The name, email and phone are mandatory fields. I am only interested in US traffic. I will only pay for US conversions. After the form is filled out it takes them to a 30 minute video. After the video is complete there is another form to complete. On the second form their is 5 fields to fill out (they are all required). I will pay for the second form as well. The website converts pretty well right now. Using adwords, FB, and 7search for conversions currently at about a cost of $3 for second form conversion (we don't currently track the first form conversions to cost only the second form). Understand that each person will be contacted by phone so we will quickly find out if you are paying people to fill out the form (lazy grey/blackhat method). Not interested in BH methods of traffic generation to my site (such as hacking FB accounts and posting on their account).
