programmer who can help with my opt in form issue (getresponse)

    hi there.i have an offer in my website where people can input their name email phone and address into a form and when they click enter these information will go into my email as a request for a free cd by them.

    i need someone to code the opt in form so that when they input their details,their email will also automatically go into my get response auto responder as well..

    urgent work so please reply asap.thanks..
