Buying Programmer to fix enquiry form in PHP/Javascript

    Hi

    I have an enquiry form made with PHP and javascript.

    Its got a couple of small bugs need fixing and I need a couple of added extras to it


    I need to

    1. Fix the bugs
    2. Add a telephone verification service - code is all intact - copy and paste job
    3. Fix the form to send visitors who fail telephone verification to a different page
    4. Send all data from enquiry form into MySQL database for easy access
