If your form uses a mysql table be sure that the connection is being established with the correct server address and login information. If it emails the information to you be sure that the server you have moved to is matching up with your code correctly. If you could post a little of the code or a specific error that you are recieving it may be easier to diagnose and fix here on the forums.
