problem for wordpress image uploading

i have a problem with upload images in wordpress. when i add media button insert media popup showing but in the popup not showing upload and media options not showing. i tried in different ways but i didnt got the solution. plzzzzzzzz rectify my problem you can see the below image

 

Quick question, are you doing this from within a Page / Post itself, or are you going to the Media section first, and then trying to upload the image?
I have seen this before with some older themes that were never updated, with newer versions of WordPress.

If you switch to a different theme (such as TwentyFifteen) does the same thing happen? If not, then the issue most likely lies with your theme.

If switching themes makes no difference, I'd recommend disabling all of your plugins and see if then the issue goes away. If it does, re-enable each plugin one by one, testing each time, and you might find the plugin that is causing the issue.
