Ok ,recieved the warning letter from adsense about
clicks. After that I checked my stats on my most recent
site and found that 25 percent of my clicks were from
one IP in China Is that the answer

    I have banned that IP

Any guidelines on what IP addresses I should ban

Seems to me that's likely the problem. If you've banned that IP address there's not much else you can do. Be vigilant from now on because if this Chinese IP address is maliciously targeting your site there's nothing to stop them coming back under a different guise to try again.

How do you know that a user actually clicked on the ad?

Google will not warn you of a click fraud if the clicks originated from the same IP.

Click fraud can actually mean other things you might think legit, like users on your forums regularly clicking on your ads without actually spending time on the site.

What kind of website is it?

    Peace,
