Well you can only be a premium publisher under certain circumstances. It's very hard to get accepted since Adsense rules a bit more loose on the "premium" publishers. This status basically grants you customized Adsense ads, a personal Adsense rep, and other goodies that normal publishers have. It's very hard to get into, but if you can, it is well worth the work and effort. My plans are to one day become a premium publisher... it takes a lot of hard work. I think you need some 2 million or 20 million impressions to even be considered into the program! *If I made any errors in the above information, someone please correct me. I wrote it based on previous information I've read around different forums.

You need 20 million impressions per month. But the reward is great, you can make your ads blend in so well with your site that it makes it very hard for people to tell the difference between an ad and a link. The CPR is usually over %15.

To add to this, 20 million expressions per month from ONE site. I'm not totally 100% on this, but those who have told me about it have said that the said 20 mil impressions must come to one site.
Yes it's supposed to come from one site. But once you have one site and become a premium publisher, you can add the extra bonus of ad editing to all of your other sites even if they have no visitors.
Is there an official guide for premium publishers? Not that any of my sites would be eligible for it, but it would be good to know, for myself.
