Pr4 Proxy - 3k~12k UV - 1,5 Month Old.

What you think about this proxy?
www.zaxiproxy.com?
It start at 25/09.
It receives at least 3k uv/day with average 5k and peak at 13k.
It have also about 3$/day via adsense and about 2euros(not bucks) with affiliate with 404 error pages.
It have also PR4 and 230k Alexa Rank(some days goes under 70k.)

Yours looks decent, but perhaps remove the Firefox banner (useless on a proxy).

If you want to change the template, use the Puzzle Proxy one... it does well for me

 

I had similar problems with proxies.

I feel that because there are so many proxies out there the adsense revenue is saturated and the cpm is low.

One thing i have done is to reduce the quantity of adsense ads - you have the max 3 + 1 link unit

Try this out it worked for me:

Replace the left adsense skyscraper ad with a widgetbucks skyscraper (select a high value electronics group {phones/computers etc} do not use merchsense)

Replace one of your square text adsense ad blocks with a image ad either

a) adsense image only or
b) adsense referals (cpa) - image only - social networking site
c) other ad network square image advert

......................

By using the above technique i more than doubled my income for a low paying proxy, the revenue from adsense has increased even though i have reduced the adsense ads.....strange but it worked for me

I also get extra money from widgetbucks - i believe they could be the next adsense - big company behind them and not a scam imho

Edit: just had another look try removing the hosting banner and the firefox banner also - i think your users may be getting so called "ad blind" because of the quantity of ads - i would put one small 468 x 60 banner on your proxified pages (maybe your hosting ad)
