AdSense isn't in trouble...far from it. Sure, Google has had to refine the program and come up with a way to thwart illegal clicks. I think their reported percentages of click fraud is too low, less than one percent. I'm betting it is closer to ten percent.

Adsense is the best way to get targeted ads on websites. For publishers it's the best/easiest way to make money off content sites. Combine those two and you have what MattKNC said: adsense is not in trouble. If worries about clickfraud slow the growth of adsense investments, that only means there are some checks on that growth. But there is still growth, and lots of it. There was news today that google actually has so much money coming in, it doesn't know what to do with it. Doesn't sound like a company in trouble to me. Adsense is a major percentage of that income.

many of the smaller programmes don't have the software and human resources to cope with click fraud or turn a blind eye to it.

I'm not saying Google is doing itself any justices with the way it treats many publishers, but its fair to say, with its financial earning capacity and spending capacity, only Microsoft and Yahoo will be able to match it for click fraud detection and for equivalent earning capacity for publishers/advertisers.
