    Yes you can approved them link but you should follow their link and if you find that their link is good enough and valuable and will helpful for your readers.
    For the spammers, i won't hate them but particularly i will place nofollow in their links.

    Blog is a good way of bring in traffic to your website and commenting on a do follow blog is great too, though no follow is common.

    Yes you are right, But you need not forget about your traffic also,
    It's nothing bad to post comment on a nofollow blog,
    use of anchor text will helpful to drive traffic towards your website.
    If you still want to get link juice from blog comment post you need to remind relevancy.
