Populate database (two tables)

    Hello, everyone!

    I need help with something very basic.

    Create a simple form like the one below:

    and submit that information into the database.

    Provide me with the php code and the code to create the database.

    What two tables are you wanting? Looks very much like a single table with 2 columns.

    Are you looking to validate the Current username? Does New Username have to be unique? Can a Current Username submit multiple New Usernames?
