Points to be kept in mind while outsourcing your link building campaign. Please feel free to add/ edit / delete and share your opinion.
I tried to put everything that I can think of. Sorry if it seem too rigid, but any link building done keeping following guideline should be a perfect.

    1. All of the links must come from content relevant web sites.
    2. All of the links must be text hyperlinks coming from static pages or the home page of the other web site.
    3. The pages containing links to web site should be reachable from the home page of the other web site within 2 clicks
    4. Links must contain the correct anchor text, that the url is typed correctly and resolves to the correct page
    5. Since link builder will be emailing on your behalf, s/he first has to get approved the content of email that s/he will be sending. S/he should also agree that there won’t be any mass mailing or any format of spam mailing. Doing so will result in cancellation of the contract.
    6. Links from sites containing hacking, cracking, phreaking, serial numbers, warez, pornographic (or other adult-relateD), hate, racism, casino, gambling, or illegal, offensive, immoral materials are NOT acceptable.
    7. Link builder will not submit site to major search engines or major directories, unless you receive direct permission from us. (Sure enough I don’t want someone else submitting my site 1000 times to dmoz catgegories and getting it banned).
    8. Links should not be from web rings or link exchange programs like Linkpartners.com.
    9. Free for all (FFA), guest books (viewbooks), links farms, blogs are absolute NO.
    10. There should not be any link from a site that seems to have incurred a Google penalty.
    11. There must not be more than 1 link on a single domain or its included subdomains. More specifically, there must be no more than 2 links from the same class C IP address (ccc.ccc.ccc.123 where the c's indicate the Class C portion of the IP address.)
    12. All of the links should be on pages with as few other links as possible. There must be no more than 50 total links on the page.
    13. The pages on which link appears must be primarily in the English (or whichever is the language of website) language.
    14. Popup or pop-under links are not acceptable. JavaScript-powered links are also not acceptable.
    15. Links must not be in discussion forums wihtout explicit permission.
    16. Links on Blog posts or only found in RSS/XML feeds are not acceptable. Permanent (not in a single posting) links on a blog may be acceptable.
    17. Links from orphan or doorway pages specifically created for links exchange are not acceptable. Final word on whether a page meets this requirement is at our discretion.
    18. A link report should be submitted in the format that contains all the following information about each link: Title of Page containing Link, URL of Page containing Link, IP Address of the Site containing the Link, Google Page Rank of Page containing Link, Text of Link, Target URL of Link.

I agree for the most part. Although I still believe that a well formed link page still has value. "well formed" is the key term though
