To comply with the recent change in Adsense "Terms and Conditions", I just created a sample "privacy policy" page in one of my site. W'd appreciate your feedbacks. Also have few questions in regards to this: a) I already have a "Terms and conditions" page for my proxy site. Can I include the privacy policy in the same page or it needs to be in a separate one? b) I currently manage multiple websites. Is it possible to have the policy text in one main site and other websites just referring to the content, thru php ".inc files". I tried to do this with the full url: http://mydomain.com/privacypolicy.inc (not sure, if .inc files work with cross domains). But it didn't work. Having it in one site would obviously save time if there is any modification or updates. http://beot.net/beot-privacy.php
As long as what you are doing is not against adsense TOS, there nothing to worry about. Good work though.

your privacy policy looks just fine. all you need to do is to setup a page with privacy policy and link from your sitemap or footer url
Well if you are displaying adsense ads in a non-english website, you could have the privacy policy in the same language, the other pages are. All you need to inform the visitor is that the third parties may be placing and reading cookies on your users’ browser, or using web beacons to collect information.
