I don't think someone will do this for free....But you have to pay someone to do that.

If you want to do it your self there is a lot of tutorials about how to create forms with php

 

I know php good but this one is not happenig at all

 

Create the form in HTML and CSS. You don't have to use only PHP in a PHP file.

 

Have a look at some basic CSS positioning and HTML forms on w3schools. As for the PHP side, you can look how to retrieve data passed from the form, but what you do with it is up to you.
