You just need to submit under the health category and the fitness section. Just submitted a site there yesterday myself. Title you might want to chose Health Tips or something along those lines, then just describe your sites point in descpription.
Everything you need to know can be found here: "How to suggest a site to the Open Directory". Anything else you might be told, especially by non-editors, is very likely to be unhelpful or even untrue.
The website's title is quite clearly shown in the title banner (not the metatag title in the title bar at the very top of the window) to be Health Tips & Guides. You might as well suggest it as that because that's how it'll be listed anyway .
Its not that important to get into dmoz anyway, many sites are not listed and do very well. I have several sites that are not included in dmoz that rank much better than dmoz listed sites. Dmoz is considered old hat now, so wouldn't bother wasting your time in getting a listing
Why do you waste time submitting to any directory, article submission, social bookmarking site, ask for links, etc. Surely DMOZ is no different. Its a link and only takes a few minutes.

How do you know that? Help with what? After you have submitted once, there is nothing more that can be done.
I have 3-5 sites in dmoz different directories. What I have seen they look for uniqueness in your site.. if your site looks like others then forget it .. it will not be approved.
That is certainly an important factor. Here is the full set of criteria that editors use when evaluating a site (wherever they find it): http://www.dmoz.org/guidelines/include.html And it's probably time for yet another reminder that DMOZ is not a listing service. Editors are encouraged to look for worthwhile sites in all sorts of places, and suggestions made by others are no more than one resource volunteers can use if they choose.

I listed my website in dmoz and wished for luck, 2 months later it was listed in dmoz allready. I guess if your website has something good to offer, they list it faster.
No. You can read the reasons here: Discontinuation of site status checks. That also explains why such a report is completely useless, because it provides no useful information at all, and merely wastes everyone's time. See other threads here for further details.
Why do you want to pay to someone to help you with category and title? If you want to pay then just go to scriptlance or many other freelance sites and buy a DMOZ listings from many editors who are selling listings. They will do all the work for you and it seems generally they can list you in about 10 days. The price just now seems to be about $100.
Er no. Most of the misguided people there are offering to pay for a listing. Not the same thing at all, and they are risking getting their sites permanently banned by doing so. I have never seen anyone on those marketplaces who actually is an editor, but nevertheless plenty of people hand over their money based on nothing more than unsubstantiated claims. There's one born every minute.
No one is as blind as the person who decides not to see. That is so funny coming from person who doesn't know the meaning of completed transaction and positive feedback.
Sorry, I have no clue what you are talking about, which is usually a good indication that you don't either. My point is very simple, but I will try to make it even more so. Many people post on those sites seeking help getting their sites listed in DMOZ. Many people place bids offering to list a site, despite not being editors. Some of those bids are accepted by gullible webmasters. The gullible webmaster loses his money and the successful bidder moves on to the next one. The sites are never listed. The world keeps turning. And still the misguided webmasters keep trying to pay for something they can't get. It astonishes me.
