Depends on what the code looks like. If its easily converted into a WordPress "feel". Also, it depends a bit on what functionality you need - a WordPress theme needs to have functionality for different features that might not be part of the original template (plugins, comments and so on. Ability to change the menu via the wp admin, sidebars and so on).

Regardless, it shouldn't take too long, I would estimate something between 2-4 hours total, depending on needs. That includes testing and updates.

Need to see it. You can probably just bang it out with something like betheme.

The pricing would depend on what you wanted.
Number of columns, number of .php pages (404.php, single_post.php, front-page.php, functions.php, header.php, footer.php, sidebar phps? etc...) (some have a lot of various page files), is there a separate mobile and PC version?, is the CSS fully cross browser or will there be browser specific tweaks?, additional functionality for the headers or ad spaces...

If you have a theme to mimic, or as a general guide, in mind it makes things easier on the programmer, and, in some cases, on can DIY it relatively easily.
