I am glad you have asked this question as i setup a directory about 4-5 month back then one day i go to login and the same problem so the site has been down for that long.
The installation of phpLD is dead simple, you can delete all the files and upload once again, or follow the instruction carefully, reinstall the script, it will be okay.

Can you post .htaccess content and apache logs when /admin page is served? (send in PM if you are not comfortable posting here).
Oops, just send the .htaccess in that case. Sometimes you may need to add/remove 'RedirectBase' line to .htaccess. I do agree with others that you will find more hands-on people on that forum.
