phpbb shoutbox problems plz sum1 help me

i have setup a forum here at http://www.chillnrock.com/

i install this shoutbox mod given here http://www.phpbbhacks.com/viewhack.php?id=1255

plz check its givin some database error...i did exatly wht was writtern in th emod...updated my DB tables too

im a noob with mysql plz help

using phpbb version 2.0.21
