I am looking for someone that can code me a script with the following criteria: This is a lead capture program with basic lead capture functions and notify options. This is what I would like the script to do. It has a interface that allows us to enter a zip code when a customer calls in and it immediately shows us if it is in our service area and what affiliate handles the area. After that we need fields that hold name and phone number and notes. When this info is entered the info needs to be saved to a mysql database and as an option emailed to a specified email address. This function makes it necessary to have a management section that allows us to add users that enter leads and a section that allows us to add affiliates with the capability to assign zip codes by .csv to each affiliate and their email where they want the lead send to. We also need a section that displays the leads for each affiliate and lets us print them out on a weekly, monthly and specified date range basis. We need to be able to edit and change all info in the management module. This is the basic idea of what I need.
