PHP MySQL Welling and Thomson Book - problem with source code on CD?

I have just started learning PHP from the Welling and Thomson book that has been mentioned on a few threads on here.

I have worked through what seemed to be a very clear first chapter. I then looked at the source code supplied with the CD that came with the book.

I can't get the very first sample to work.

Is this a known problem, or do I need to look at something in my set up? I have the fourth edition so I am assuming that I am doing something wrong.

The specific problem seems to be that the variables assigned on the html page aren't being passed to the php script to be worked on.

The code on the CD for Chapter 01 isn't very helpful. I had assumed that it was the final state the code should be in if you had worked through the chapter. In fact it doesn't have all the updates that you work through, specifically it misses out the code added on page 25.

If you are trying to get through the intro as quickly as possible like me I think it is quite likely you would make the same mistake.

I have now moved onto Chapter 2, and there isn't any source code on the CD at all, even though it is referred to in the text. Is a pattern emerging here? More practically, does anyone know if the missing info is on the web anywhere?
