PHP/Java order form editing quick simple ><

    Hey guys :-(

    I am trying to Add a New field and colum to this current php order form script : http://www.dyn-web.com/scripts/orderform/

    The colum is going to be called "type" would like it to be a drop down menu containing the options a-j where a = $5 b = 10$ c = $15 d=$20 etc etc..

    example: if drop down menu b is selected on product 2 it will add $10 to the total boxes.

    I only really know html and can install php scripts thats about it :-/
    i would post in the service section but i need 25 posts in order to do so. Paying 15 - 20USD to anyone who can help successfully (if allowed)

    Im going to try to work on it, but im less then proficient with javascript, I'll see what I can do..
