Php form help - Will pay 30-40$ - Urgent

    Hello I need some help with a form. There are 2 upload fields and 2 text boxes basicly that i need help with. I want to select 2 files and upload them and then want the save the text in the textfields as text files.

    Some more options too, but the help i need largely is for the above. But need it quick.
