Php Contact form

    Hello, i wonder if someone could be nice to make or post a script for a php Contact form that i need to my website.

    What is needed in the form is that the user have to post hes name or username and email, "this is requierd or he can send the mail trou the form"

    and should have multiple choices to where it's sent "webmaster" "marketing" and more etc.

    and ofcourse the message. this email should be send from let's say to > the email that he wanted it sent to, for example marketing (marketing@mydomain.com)

    i hope someone know a script of this kind of would be really nice to make one.

    ye i had a look at hotscripts, and what i found was only form wich suport 1 email adress only for all subchoices :/
