Buying PHP Contact Form w/ Validation

Hello,
I need a general php contact form that I can use on any website that I want.

the form needs to have the following elements
Regular input field
Regular input field that is required
Email input field (validate field and required)
comments field (required)
phone number field (validate input format 111-111-1111)

And any other general fields that you think might be useful.

Please send me your price and turn-around-time.

I'm going to pay via PayPal. But before payment, please upload the form to your server, so that I can test it. Once I approve, then I will make full payment.
