PHP Contact form needed.

Hi,

I need a simple php contact form, but to be honest my php sucks so I'm hoping that a dp member could help me out.

It just needs to collect the name/telephone number and a comment from the user & email them to site adminstrator. Nothing big is needed, can anyone direct me to a prewritten script for this kind of thing.
