Buying PDF to HTML/CSS coder basic job $30

    I'm looking for someone to convert a PDF into an html document.

    The pdf itself is a form but I want the html document to be static (no inputs) as it is to be converted back into a better looking pdf.
    The form is a single A4 sheet.
    I would like it styled like the pdf but with an expanded width to fill the A4 page.

    Please show previous works.
