It depends from one to another, some are extremely picky about what they will let in regardless of you paying, others are a bit more lenient and some will let literally anything in as long as you pay.

I my opinion to pay for directory submission is not effective now. Maybe except yahoo and DMOZ. Google do not pay so much attention to the links from directories like it was in the past.
Yes, like freelistfool said, most directories I know will refund the money you paid for review, if your link is not accepted. However, to guarantee that refund, first make sure the directory you're submitting to clearly states that. Or you can equally ask before submitting your link.
