Your internal pagerank is always being adjusted based on the amount and quality of links you have coming in, your internal site linking, etc. Toolbar PR is just the green ego booster that changes every 3-4 months to show the world what the PR of those pages actually is in Google's eyes.
Pagerank is the actual number of your page rank that is only known by google. and Toolbar PR displays your PR as a whole number ranging from 0-10 based on your "actual PR"
Toolbar PR is not accurate. First off it updates once in every couple of months, second off, it might decrease if you have too many outgoing links or if you are selling links, however, your SERP will likely remain still, which means you don't lose any rank but just looks such.
