    Hey All

    I need a packing slip designed for my site.

    Specifications,

    Must be designed to fit http://pcforms.com/peel-shipping-label-sheets-lsa5l-p-271.html

    In the Peel off Label

    -My Logo & Adddress
    -Buyer Address

    On the Rest of the paper must have:

    -My Website Address
    -My email Address
    -Date Order
    -Payment Method
    -Delivery Address
    -Billing Address
    -Customer Notes section
    -order number
    -Quantity
    -Product
    -Price Each
    -Total Price
    -Total Price of all Products
    -Shipping
    -Grand total
    & must have a text area where I can add text.

    I need it designed & then coded into my zencart site.(easy job)

    Will pay $10 to winner.
