Just wrote an article called It's all about the image where I share my experiences in image optimization. From what I have seen, surrounding text is what counts the most when it comes to ranking for keyphrases in Google images.

Actually, in my experience the image name (I know it sounds too easy to be true) carries a lot of weight as well. Yes, but compared to the results for top keywords in the regular results, image results don't seem to have a sandbox.
