I wouldn't personally use a self-hosted script for several reasons as there are cheap and reliable alternatives. The script is called Infinite responder. Its pretty easy to set up and free, You need hosting for the script on your domain. Biggest downfall is deliverability, I find with this solution a load of emails don't get to the recipient as your domain is not whitelisted. Its like trying to get them to subscribe a second time. I have had success with that script and still use it as the registration form my software products.
