    On page factors:

    Keywords on title tag.
    Keywords on content.
    Keywords on headers.
    Internal linking based on keywords.

I do not recommend any tool for this. Use your common sense.
A good tip is to make look natural your website.

Check the placement of keywords through various html tags like title, Meta keywords, Meta description, header tags, alt, list, text formatting and others. Then tweak html code for css, javascript, frames and other seo barriers.

Basically onpage optimization factors can be divided into the two main branches: the factors that shape the structure of a website and the factors that help you to make up the the content for your site.

    Website structure factors are:

-Domain: Before you choosing a domain name one'd better know their target audience, and what they intend to sell. Then try using the major keywords in a domain. Using a .com version of a URL is better than other versions.
-Contact page: Design a contact page and put your tel.number, E-mail and mail address, icq, skype etc.
-FAQ: design a Frequently Asked Question section. I think EVERY website needs that.
-About Us. For visitors to know who you are.
Robots.txt file: With robots.txt file you can limit access of search engines to your website.
Site map page: helps visitors and search engine bots find pages on the site.

    Content Optimization Factors

-Page Title: use your keywords on page title, the closer to the beginning of the Title the better.
-Use <h1> header tags: put the topic in <h1> header tag.
-Keywords Meta Tags -useful search engine spiders
-Meta Keywords tag -a brief and concise summary of your page's content.
-Add <alt> image tags: use Alternative Image Text.
-Use <h2> header tags for sub heading.
-Keyword Density: The percentage of times a keyword appears on your page, compared to the total number of words on the page.

    That's it
    :)

On-page factors include anything that you can affect on the page itself, such as the title tag, body copy, H1 heading tags, image ‘alt’ attributes etc.

Off-page factors encompass things that influence rankings but are not on the page itself, such as PageRank score and anchor text, better known by the layman as link text.

When you are optimizing the on-page factors, you start with keyword research to identify the keyword markets that you should go after. You revise and add content targeted around those keyword markets (content optimization) and you optimize your HTML templates so that the content that is contained within has its best chance to sing to the search engines.

I recommend you to use a Firefox Add-on - SenSEO.
"SenSEO is an extension checking the most important on-page-SEO-criteria and calculating a grade of how good your site fulfills these criteria."
    Is great and is free.
