On Content - Do you start with keywords then placements?

    I'm very good in search side of things, however for content I need help. How do you folks start your campaigns on content?
    1) Keywords (text)
    2) Placements (url)
    3) keywords + Placements (text on specific page - the keyword has to appear on the page)

    Also, do you use PPC Bully to help you and if yes then do you focus on placements?
