There are bots as well as humans that review websites periodically to make sure that your website is in compliance with their TOS.
Google is always going to battle against spammers and low quality websites. Sometimes those spammers cross the line and Google completely loses trust in that website. At that point it is not worth giving that website 2nd and 3rd chances. Google assumes that certain infractions are a sign that that website will always be untrustworthy. So rather than have a website constantly setting off manual review filters, it is much more efficient to ban the website. Then they don't have to deal with that website unless they come begging to get back in Google via a reinclusion request. For smaller infractions a website usually gets a ranking penalty of a certain number of positions. If you do something spammy, it is common for your website to drop from page 1 to page 5. More serious infractions could put you past page 10.
The entire Google system is based on algorithms and flags, if a site shows unusual activity it may be flagged for a real person to check it out. Usually Google bans sites that are misleading or harmful. If you stick to the rules you wont have anything to worry about.

Google has a set of rules that they expect all sites to follow and whenever someone breaks those rules they will get banned, if google didn't ban sites there would be much more spam on the internet.
