    Are you talking about the on-page popup out box? Check out GreyBox.

    You will still need the form/autoresponder as the last poster mentioned.

    If your talking about the pop up box, look at thickbox. Thickbox also has much more styling functionality and looks better!

    Welcome! Just tryin to help. Also, it has the option of including html inside, unlike simmilar jquery pop up boxes like lightbox (which i use for images)
