Buying Need Wordpress or HTML feedback form (s) Simple jobs.

    I am looking to have 2 simple feedback forms done. My domains run wordpress now so the feeback form could either be done with wordpress plugin or by hard linking an html page to a pager header link on my wordpress main page.

    This has the information that I want customers to input but its way way too ugly and it just may be easier for me to have this done in html as I will want to use the same form on all 53 of my domains.

    what is your budget.................the form can be designed in html but it will work with php script.....to receive the form fields values
