Buying Need Website Built (design & Coded) - Simple 5 Page Site

This is a simple, maybe 5 page website for a lawyer. It is basically going to be a simple, static, brochure type website. It should be clean and professional looking and look like it was designed in 2013 not 2000. Nothing fancy, but not something so basic it looks old.

I have all the content. I will need a single form on the website where people can enter their information which will then be emailed to the attorney. I want the design made so that I can easily add an additional page should the need arise.

I am interested in seeing some sample work from designers and some quotes for the project. If you don't have something like this in your portfolio, then show me a website you think fits the description that you could make yourself. Understanding the design goal is half the battle. (I have been looking at designers' portfolios but everything is for much more complex sites and on totally different looks. I can appreciate the skill shown, but want to know that someone understands this project)

Feel free to ask any questions. Eventually I can show his website to the designer and other sample site ... but I think would rather see what designers here think is good conservative look for a lawyer's website.
