Buying Need web based too to do a lookup

Hi all,

There is a website where you can submit some info (serial number) and it will tell you when your subscription to its services will expire. It uses a form the then moves to a results page.

I need to be able to have a form on my website that will submit the info in the background, grab the result and then show on my website, kind of white labelling the process in a way.

I would prefer PHP and/or JavaScript if possible.

Please try to reply with price and time frame, I will answer any questions.
