Need very simple script made to insert data in DB

Hi there guys, I need a simple script made where I can fill in a form, and submit it into a MySql DB. Data would be similar to the data below.

do you need a form ? and when you submit it goes to mysql DB. ..do you need any validation required during form submission ? kindly explain.
