"Build Your Own Web Site The Right Way Using HTML & CSS, 2nd Edition" by Ian Lloyd
"The CSS Anthology, 2nd Edition" by Rachel Andrew
    "Simply SQL" by Rudy Limebrick

All books from SitePoint... sorry, can't help you with PHP (can't find the link to the online wiki I keep forgetting to bookmark)

The PHP manual is a great way to learn PHP. Install something like XAMPP, and just start making 'stuff'.

I used to use XAMPP, until I "upgraded" to WampServer.

HTML-Basic web design, foundation of principles you will need to understand. It is the meat and bones of your website, and whether or not you are relying on CSS, or possibly some user interface for designing your web page, there will come a time when you need to use basic knowledge of HTML. Also, this would include constructing the code in between the <HEAD> tag of your website, which changes depending on the code you're using, programs you're using and important for meta tags.
