I want to make a webform similar to following form http://bit.ly/R6E8wd my requirement is that in reponse of the question "What product(s) are you interested in?" , there are 5 options from which 1 can be selected.. on form submit, the user should be redirected to different url based on the selection e.g ProductA or ProductB etc etc ... if user selects option 1(product A), he should be redirected to url1 if user selects option 2(product B), he should be redirected to url2 and similarly if user selects option 3(product E), he should be redirected to url3 also all the form fields should be emailed to my defined email address. the form should be very neat and clean and should look very professional. pm me your price range for this work and also your expertise.
