Buying Need someone to program a one page form website with email - $30

    I need someone to make a one page form website for a unlocking service I have. Pretty much all I need is for this to happen. User enters his phone model, email address, his imei, and selects his carrier. Then it tells him the price. Then he does a express check out to paypal. After he pays using paypal a email will be sent to the buyer and me with the information submitted. Using existing scripts is ok and I have a design already if your doing it from scratch. Willing to pay $30.
