need someone to link my opt in box to my get response autoresponder..urgent..

    simple job..as said above..i have a opt in box template..i need someone to link it to my get response autoresponder so that when my page loads..5 secs later the opt in box appears..and when people input their details(name and email) and click submit,they are redirected to another page..simple job for those who know how..need it done urgently..pm me..
