Need Someone To Install A 'Contact Us' Form

    I need someone to install or build a contact us script (similar to http://turnkey-buddy.com/store_contact/)


    The 'Contact Us' form needs to be:

    - Put on to this page http://www.websitescripts.org/_contact

    - Needs to be 'integrated' into this page, cannot open on a separate php file.

    - I need to receive the email in my inbox like it is being sent from the user.

    - Needs a SPAM feature (captcha)

    - Should have a drop down menu function.


    This is probably a really easy job for someone if they have the script already.

    I need this done ASAP. I will pay someone $10 for just installing the script.
