    What is it that you need done?

    Creating a form?
    Please can you add more information or PM me with the details as I'm very experienced with Wordpress plugins and can do this job. Thanks

    i can complete you job , i am experienced wordpress user and also did lot of customization for wordpress and also a wordpress theme coder , i need few minutes to setup this plugin for you my price for this work is $10 and i use paypal to take funds for my work.
