Need sliding ad designed and coded

    Hey,

    I need something similar to the bottom ad here
    Where the opt-in form is.
    Just that I want a few things extra - search box, rss email form, an ebook cover, twitter following, some banners.
    Anyone is up for the challenge? The reward is quite decent. :)
