Need simple one page for my site

    Need a simple page doing for a site of mine. Basically its just a contact page where users enter name, dob , mobile number and email and it gets forwarded to my email and needs area for me to add text.
    Needs to seo freindly and possible adsense ready.
    Not looking to spend much, no fancy graphics etc are required.
