Need Several Forms (4 to be exact)

    Hello,

    I need 4 forms for my webhosting company, these forms are going to be used for clients to send in some information regarding add-ons and requests to their webhosting accounts...

    The 4 forms needs Required Fields and Not Required Fields.. This is something that i need to be able to decide! Also, the mail the user enters needs to be submited as the "sender" of the mail...

    The title of each form i must be able to chose, the 4 forms is going to be sent to 3 different mails! Basicly 1 form to 1 mail and 2 forms to 1 mail and so on! (Not send the 1 submited mail 3 mails)

    I hope someone out there can help me asap, reward? 5$ per form :) Send me an PM or post here.
