Need Quick PHP Job Doing

Hi,

Im looking for a very very simple tell a friend script. I have a box on my site im doing that looks like this-



Im not looking for anything fancy, nice and simple there are only 2 input fields, friends name and e-mail address and want it to send a pre-defined message.

There are lots of free scripts available to do this and i would be happy if someone could just modify one for my needs, examples-
