Need Programming Assistance

    I want to make a sort of customer questionnaire, where, lets say 100 customers are asked around 10 questions, and then the results will be in a graphic representation.

    Actually quite simular to the poll feature on this forum, but i need to have a bit more options to cover the questions and answers.

    Maby there is a sort of template that i eazy can use for this purpose, or do i need to have it special maed for me ?

    Any help will make me wery happy....
